How to Complete It
This option is for students who either:
- Have already been admitted to a degree-seeking graduate program and are adding a master's or PhD along the way, or
- Are adding a second master's degree as part of an approved dual degree program
In the latter case, the second program should have approved the student for admission internally before submitting the form. See the graduate admissions guide for staffÌýfor details.
- Enter the effective term for the change. Changes can only be made for the current or future term within deadlines as posted above.Ìý Ìý
- Please note: If a master’s degree program is added in the same degree program as an existing PhD program, the original admit date will be used.ÌýIf a PhD program is added to an existing master's degree program, the admit term will be changed to the new term.ÌýIf a new degree program is added, the new admit term will be used.
- UsingÌýthe "Add" columnÌýonly, enter all information for the new degree program.
- Enter all signatures (ISSS, department, chair, etc.) as required.
- Submit the form toÌýIUT@colorado.eduÌýfor processing.
This option is for students who are removing a master's or PhD program, but intendÌýto remained enrolled in another degree-seeking graduate program. If the student is withdrawing from all programs, refer the student to theÌýwithdraw from the semester webpage.
- Enter the effective term for the change. Changes can only be made for the current or future term within deadlines as posted above.
- Please note: The admit term of the remaining program will not be changed and the time limit for completion will be accounted for by the original admit term.
- If you want a different requirement term for degree audits, please indicate in parentheses next to the effective term. For example: Fall 2020 (requirement term 2197).
- Using the "Remove" column only, enter all information for the PhD or master's degree program being removed.
- Enter all signatures (ISSS, department, chair, etc.) as required.
- Submit the form toÌýIUT@colorado.eduÌýfor processing.
This option is for students who are changingÌýfromÌýa master's program to a PhD program.
- Enter the effective term for the change. Changes can only be made for the current or future term within deadlines as posted above.
- Please note: The admit term will be changed to the new term (students may not have met the PhD admissions requirements at the time of admission to the master's degree).Ìý
- If you want a different requirement term for degree audits, please indicate in parenthesis next to the effective term. For example: Fall 2020 (requirement term 2197).
- In the "Add" column, enter all information for the PhD degree program.
- In the "Remove" column, enter all information for the master’s degree program.
- Enter all signatures (ISSS, department, chair, etc.) as required.

This option is for students who are changingÌýfromÌýa PhD program to a master's program.​
