Fetal Dosimeter Application

Use the fetal dosimeter application to officially declare a pregnancy for anyone who uses radiation as part of your job on campus. Declared pregnant radiation workers fall under a lower allowed occupational radiation dose limit for the duration of the pregnancy and receive regular reports of exposure (even if none is noted).

This program is completely voluntarily.

  1. Fill out the fillable PDF application with as much information as possible after completing Radiation Safety Training.
  2. Print out form
  3. Return form to the Radiation Safety Office via email (radsafety@colorado.edu), campus mail (413 UCB) or fax to 303-492-1322
